Yellow-wort, or Blackstonia perfoliata. Grows on lime-based soils, particularly around coasts - this is from a limestone cliff in Wales.

Seems to have a bit of trouble getting any contrast into all yellow flowers - any hints from anyone??
The Yellow-wort species name of perfoliata means "through the leaf", and that's what the stem appears to do. Here's a stem doing just that -

With "wort" in the name it should have some medicinal use, but I haven't been able to find out what.
